Who's to blame for the hyper-partisan atmosphere created in this country?

Everything from Obama being heckled to a democratic congressman accusing republicans of genocide to another senator claiming anyone who opposed national health insurance to be an Aryan nations militia member...how did things come to this and why are they escalating?
